Passport Acceptance Center

The City of Carnation processes routine United States DS-11 Passport applications. DS-11 forms are used for Children’s passports, first-time Adult passports and special renewals (passport issued more than 15 years ago and lost or stolen passports).

Please note: The standard adult renewal (DS-82 form) is mail-in only and we do not process them at our facility.

If you have a life or death emergency and need to travel within 30 days, please contact the Department of State for an appointment : 1-877-487-2778 .

Passport photos are available for $20.

Drop-In hours are Monday, Tuesday, Thursday and Friday from 9 a.m. to 12 p.m.
